Our services are initiated through hospitals, hospices, and care homes. They will identify a patient who can benefit from a comprehensive home-based care service.
Coming Home takes the stress out of the transition to your home from the moment you are discharged from the hospital. We will work together until you are able to function independently.

Why do we do it?
From my experience (having a comprehensive medical aid), I received little advice/ support from the hospital once I was discharged.
My husband and extended family had to do research about home care. They didn’t have the knowledge and skills to assist me. As a result, they felt inadequate and I felt misunderstood.
Being a caregiver and a survivor means I have lived on both sides of the spectrum. I have a wealth of knowledge to share with those in need.
